Our Services

Person-centered supports available through Indiana Medicaid Home and Community-Based Services (HCBS) Waivers. Every service is individualized around each person's goals, strengths, and Person-Centered Individualized Support Plan (PCISP).

Residential Habilitation & Support (RHS)

Individualized supports that help people live safely and successfully in their home while building the skills needed for greater independence — focused on daily living, community participation, and personal goals. Our staff encourage individuals to learn and practice skills rather than doing everything for them.

Participant Assistance & Care (PAC)

Hands-on assistance with daily activities that help individuals remain healthy, safe, and as independent as possible. Staff provide only the level of assistance needed, encouraging individuals to complete as much of each task independently as they can.

Day Habilitation

Opportunities to build daily living, social, and communication skills through meaningful activities — selected around each person's goals and interests, with community participation encouraged whenever appropriate.

Wellness Coordination

Coordination and monitoring of health-related needs according to each person's service plan. It doesn't replace your doctor — it supports communication and coordination with your healthcare providers, with families included (with appropriate consent).

Respite

Temporary support for individuals while giving primary caregivers planned or emergency relief. Individuals continue participating in meaningful daily activities while caregivers receive time to rest or attend to other responsibilities.

Transportation

Safe travel to approved waiver services, community activities, appointments, employment, and volunteer opportunities — helping individuals participate in their communities and maintain relationships.

OUR STAFF

Trained for every individual.

Every employee completes comprehensive orientation and ongoing education, plus individualized training on each member's PCISP, communication preferences, health needs, and emergency procedures. Our training program is developed and overseen by our Psychiatric Mental Health Nurse Practitioner.
